Incident Cause Analysis Method

The ICAM Investigation process has 6 steps:

1. Gathering and Preparing Evidence

2. Identify Absent / Failed Defenses (last-minute measures that should have stopped the incident)

3. Identify Individual / Team Actions (errors or violations made by personnel)

4. Identify Task / Environmental Factors (conditions that existed at time of the incident)

5. Identify Organizational Factors (underlying organizational factors)

6. Develop Recommendations
